مقدمه
در بخش اول ابتدا در مورد پنجمین راهنمای تشخیصی و آماری اختلال های روانی و اختلال های اضطرابی مطالبی بیان میشود. سپس اختلال های اضطرابی فوبی خاص، اختلال وحشت زدگی، اختلال اضطراب فراگیر، اضطراب اجتماعی و آگورافوبیا با ذکر ویژگی های تشخیصی، شیوع و سبب شناسی ذکر خواهند شد. در بخش بعدی از این فصل مفهوم شخصیت، دیدگاه های مختلف شخصیتی، سرشت و رویکردهای زیستی شخصیت بیان گردیده است. همچنین در ادامه نظریه های مطرح شده در فصل اول، نظریه آیزنک، نظریه پنج عامل بزرگ، نظریه گری، نظریه کلونینجر و مدل سه بخشی واتسون شرح داده خواهند شد و در پایان، مدل مطرح شده در این پژوهش، مدل ترکیبی سرشت عاطفی و هیجانی بیان میشود و در آخر به تحقیقات مربوط به نظریه های مطرح در بالا و بررسی اختلال های اضطرابی با توجه به نظریه های زیستی اشاره میشود.
راهنمای تشخیصی و آماری اختلالهای روانی (DSM)
انجمن روانشناسی آمریکا (APA) برای نخستین بار نسخه ی اولیه ای از DSM را در سال 1844، به عنوان طبقه بندی آماری از بیماری های روانی منتشر کرد. این نسخه به منظور بهبود ارتباط متخصصان درباره بیماران مختلفی که در بیمارستان های روانی نگهداری میشدند، طراحی شد. بعد از جنگ جهانی دوم، DSM از طریق چهار ویرایش اصلی صورت گرفته، به یک سیستم طبقه بندی تشخیصی برای روانپزشکان، پزشکان و سایر متخصصان بهداشت روانی تحول یافت که ویژگی های اساسی طیف گسترده ای از اختلالات روانی را توصیف می کند (انجمن رواپزشکی آمریکا، 2013).
اولین ویراست DSM انجمن روانپزشکی آمریکا که اولین راهنمای رسمی روانپزشکی برای توصیف اختلال های روانی بود در سال 1952 انتشار یافت (هالجین و ویتبورن، 1389). DSM یک نظام چندمحوري است، محور مجموعه اطلاعات مربوط به جنبه ای از عملکرد فرد است. به اين معني كه هر فرد مبتلا به اختلال های رواني را ميتوان بر اساس پنج محور كاركردي مختلف كه هر كدام نماينده ی جوانب گوناگوني از زندگي شخص اند، بررسي كرد. محورهاي يك و دو نمايندهی نشانگان باليني هستند. محور سوم بیماری های جسمانی فرد را مشخص میکند كه ممكن است رفتار فرد را تحت تأثير قرار دهند. محور چهارم، مشكلات روانی- اجتماعی و محيطي مانند رويدادهاي ناگوار زندگي را پوشش مي دهد. محور پنجم، ارزيابي کلی عملکرد فرد از لحاظ رواني، اجتماعي و شغلي است (هالجین و ویتبورن، 1389). در جدیدترین ویرایش DSM اختلال روانی به عنوان سندرمی تعریف شده است که با نابهنجاری های معنادار بالینی در شناخت، تنظیم هیجانی یا رفتار فرد مشخص میشود. این دسته از اختلال ها نشان دهنده ی نقص عملکردی در فرآیندهای روانی، زیستی و یا رشدی می باشند که زیر بنای کارکردهای روانی هستند. اختلال های روانی معمولاً با پریشانی یا ناتوانی قابل ملاحظه در فعالیت های اجتماعی، شغلی یا دیگر فعالیت های مهم مرتبط می باشند (انجمن رواپزشکی آمریکا، 2013).
تعریف اضطراب
اضطراب به منزله بخشی از زندگی هر انسان، در همه افراد در حدي اعتدال آمیز وجود دارد و در این حد، به عنوان پاسخ سازش یافته تلقی میشود. به عبارت دیگر، می توان گفت که اضطراب در برخی از مواقع، سازندگی و خلاقیت را در فرد ایجاد می کند، امکان تجسم موقعیت ها و تسلط بر آنها را فراهم می کند و یا آن که وي را بر می انگیزد تا به طور جدي با مسئولیت مهمی مانند آماده شدن براي یک امتحان یا پذیرفتن یک وظیفه اجتماعی مواجه شود. بالعکس، هنگامی که اضطراب مرضی وجود دارد ممکن است جنبه مزمن و مداوم بیابد که در این صورت نه تنها نمی توان پاسخ را سازش یافته دانست، بلکه باید آن را به منزله منبع شکست، سازش نایافتگی و استیصال گسترده اي تلقی کرد که فرد را از بخش عمده اي از امکاناتش محروم می کند و تنها بر اساس راهبردهاي محدود کننده، آزادي و انعطاف فرد کاهش می یابد و طیف گسترده اختلال هاي اضطرابی را که از اختلال هاي شناختی و بدنی تا ترس هاي غیر موجه و وحشتزدگی ها گسترده اند، را به وجود می آورد (دادستان، 1380).
اضطراب حالتی است که با احساس وحشت مشخص می شود و با علائم جسمی که بیانگر افزایش فعالیت دستگاه اعصاب خودکار میباشند (مثل تپش قلب و تعریق) همراه است. اضطراب بر عملکرد شناختی اثر گذاشته و موجب تحریف هاي ادراکی میشود و وجه افتراق آن از ترس این است که ترس پاسخ متناسبی به یک تهدید شناخته شده است، در صورتی که اضطراب، پاسخ به تهدیدي است که ناشناخته، مبهم، یا متعارض میباشد (کاپلان و سادوک، 1392). اضطراب و اختلال های اضطرابی تأثیر زیادی بر عملکرد روزانه فرد، تعاملات با محیط و ارزیابی های شخص از حالات درونی خود دارد. این تأثیر بخصوص اگر اضطراب برای اولین بار در کودکی و نوجوانی پدید آید می تواند مخرب باشد، زیرا بروز زود هنگام آن با افزایش در شدت و طول عمر اختلال مرتبط است (پرزادگار و فاکس، 2005).
DSM- 5 و اختلالهای طبقهی اضطرابی
DSM- 5 شامل طبقات تشخیصی زیر میباشد: اختلال های عصبی- رشدی، اسکیزوفرنی و سایر اختلال های سایکوتیک، اختلال های دو قطبی و اختلال های مرتبط با آن، اختلال های افسردگ، اختلال های اضطرابی، اختلال وسواس فکری- عملی و اختلال های مرتبط با آن، اختلال های مرتبط با آسیب و عوامل تنش زا اختلال های تجزیه ای، اختلال های علائم بدنی، اختلال های تغذیه ای و خوردن، اختلال های دفعی، اختلال های خواب- بیداری، اختلال های جنسی، اختلال های ملال جنسی، اختلال های رفتار ایذایی، کنترل تکانه و سلوک، اعتیاد و اختلال های مرتبط با مواد، اختلال های عصبی- شناختی، اختلال های شخصیت، نابهنجاری های جنسی، سایر اختلال های روانی، اختلال های حرکتی و عوارض جانبی آنها، وضعیت هایی که ممکن است کانون توجه بالینی باشند (انجمن روانپزشکی آمریکا، 2013).
